ABSTRACT Purpose To investigate whether there are differences in cepstral and spectral acoustic measures between women with behavioral dysphonia with and without laryngeal lesions and verify whether there is a correlation between such measures and the auditory-perceptual evaluation of voice quality. Methods The sample comprised 78 women with behavioral dysphonia without laryngeal lesions (BDWOL) and 68 with behavioral dysphonia with laryngeal lesions (vocal nodules) (BDWL). Cepstral peak prominence (CPP), cepstral peak prominence-smoothed (CPPS), spectral decrease, and H1-H2 (difference between the amplitude of the first and second harmonics) were extracted. They were submitted to the auditory-perceptual evaluation (APE) of the grade of hoarseness (GH), roughness (RO), breathiness (BR), and strain (ST). Results BDWL women had higher H1-H2 values and lower CPP and CPPS values than BDWOL women. More deviant voices had lower CPP and CPPS values. Breathy voices had lower CPP and CPPS values and higher H1-H2 values than rough ones. There was a weak negative correlation between CPP and RO, a moderate negative correlation with GH, and a strong negative correlation with BR. CPPS had a moderate negative correlation with GH, RO, and BR. H1-H2 had a weak positive correlation with BR. There was a weak positive correlation between spectral decrease and ST. Conclusion H1-H2, CPP, and CPPS were different between BDWOL and BDWL women. Furthermore, cepstral and spectral measures were correlated with the different APE parameters.

RESUMO Objetivo Investigar se existem diferenças nas medidas acústicas cepstrais e espectrais entre mulheres com disfonia comportamental com e sem lesão laríngea, bem como verificar se existe correlação entre tais medidas e o julgamento perceptivo-auditivo da qualidade vocal. Método Participaram 78 mulheres com disfonia comportamental sem lesão laríngea (DCSL) e 68 com disfonia comportamental com lesão laríngea (nódulos vocais) (DCCL). Foram extraídas as medidas CPP (cepstral peak prominence), CPPS (cepstral peak prominence smoothed), declínio espectral e H1-H2 (diferença entre a amplitude do primeiro e do segundo harmônico), assim como o julgamento perceptivo-auditivo (JPA) do grau geral de desvio vocal (GG), graus de rugosidade (GR), de soprosidade (GS) e de tensão (GT). Resultados Mulheres com DCCL apresentaram maiores valores de H1-H2 e menores valores no CPP e CPPS, em relação às mulheres com DCSL. As vozes mais desviadas apresentaram menores valores do CPP e CPPS. As vozes soprosas apresentaram menores valores de CPP e CPPS, assim como maior valor de H1-H2 em relação às vozes rugosas. Houve correlação negativa fraca entre o CPP e o GR, negativa moderada com o GG e negativa forte com o GS. O CPPS apresentou correlação negativa moderada com o GG, GR e GS. A medida H1-H2 apresentou correlação positiva fraca com o GS. Houve correlação positiva fraca entre o declínio espectral e o GT. Conclusão As medidas acústicas H1-H2, CPP e CPPS apresentam diferenças entre mulheres com DCSL e DCCL. Além disso, há correlação entre as medidas cepstrais e espectrais e os diferentes parâmetros do JPA.

ABSTRACT Purpose To compare the efficiency of different vocal self-assessment instruments for dysphonia screening. Methods 262 dysphonic and non-dysphonic individuals participated in the research. The mean age was 41.3 (±14.5) years. The diagnosis of dysphonia was based on the auditory-perceptual analysis of the sustained vowel “é” and on laryngological diagnosis. The responses of the instruments were collected: Voice-Related Quality of Life (V-RQOL), Voice Handicap Index (VHI), VHI-10, Voice Symptoms Scale (VoiSS), and the Brazilian Dysphonia Screening Tool, (Br-DST) called in Brazilian Portuguese Instrumento de Rastreio da Disfonia (IRDBR). To analyze assertiveness in relation to the presence of dysphonia, the cutoff points of each instrument and the decision rule recommended by the IRDBR were used. An exploratory analysis was performed to compare mean scores of instruments and verify associations between variables. Results The instruments evaluated were sensitive to capture the impact of dysphonia in a similar way regardless of professional voice use and type of dysphonia. There was a difference only in VoiSS scores for the variable gender, with a higher score for females. Regarding global assertiveness, the instruments showed high rates of success in classification, with emphasis on the VoiSS, which had the highest rate (86.3%), followed by the IRDBR (84.0%), VQL (80.9%), VHI (78.2%), and VHI-10 (75.2%). Conclusion The VoiSS has the highest assertiveness index in the identification of dysphonia, followed by the IRDBR. The IRDBR is a short, simple, and easy-to-apply tool for screening procedures.

RESUMO Objetivo Comparar a eficiência de diferentes instrumentos de autoavaliação vocal para o rastreio da disfonia. Método Participaram 262 indivíduos disfônicos e não disfônicos, com média de idade de 41,3 (±14,5) anos. O diagnóstico da disfonia foi dado a partir da análise perceptivo-auditiva da vogal sustentada “é” e do diagnóstico laringológico. Foram coletadas as respostas dos instrumentos: Questionário de Qualidade de Vida em Voz (QVV), Índice de Desvantagem Vocal (IDV), IDV-10, Escala de Sintomas Vocais (ESV) e do Br-DST (Brazilian Dysphonia Screening Tool), denominado no português brasileiro como Instrumento de Rastreio da Disfonia (IRDBR). Para análise da assertividade destes em relação à presença da disfonia, foram utilizados os pontos de corte de cada instrumento e a regra de decisão preconizada pelo IRDBR. Foi realizada uma análise exploratória para comparação das médias dos escores dos instrumentos e verificação de associações entre as variáveis. Resultados Os instrumentos avaliados foram sensíveis para captar o impacto da disfonia de forma semelhante independentemente do uso profissional da voz e tipo de disfonia. Foi observada diferença apenas nos escores da ESV para a variável sexo, com maior pontuação observada no sexo feminino. Em relação à assertividade global, os instrumentos apresentaram elevados índices de acerto na classificação, com destaque para a ESV que apresentou maior índice (86,3%), seguida do IRDBR (84,0%), QVV (80,9%), IDV (78,2%) e IDV-10 (75,2%). Conclusão A ESV apresenta maior índice de assertividade na identificação da disfonia, seguida do IRDBR. O IRDBR é uma ferramenta curta, simples e de fácil aplicação para procedimentos de rastreio.

ABSTRACT Purpose to map the vocal assessment measures used to verify the effect of the intervention in vocally healthy individuals. Research strategy This is a scope review based on the research question: What vocal assessment measures are used to verify the effect of the intervention in vocally healthy individuals? The search was carried out electronically in MEDLINE (PubMed), LILACS (BVS), Scopus (Elsevier), Web of Science (Clarivate), Embase and Cochrane databases. Selection criteria The selection of studies was based on reading the titles, abstracts, keywords and full texts, applying the eligibility criteria. Data related to the publication’s bibliographic information, sample and intervention characteristics, the effects of the intervention on self-assessment, auditory-perceptual, acoustic, aerodynamic, electroglottographic measures, laryngeal examination results, among others, were extracted. The data were summarized and presented in a quantitative and descriptive way. Results 97 articles were selected, among the 979 studies mapped in this review. Acoustic analysis was the most used measure (n=70, 72.3%) to verify the effects of vocal training in the selected studies, followed by electroglottography (n= 55, 56.7%), self-assessment (n= 38, 39 .2%), aerodynamics (n= 33, 34.0%), auditory-perceptual judgment (n= 22, 22.7%) and laryngeal examination (n= 16, 16.5%). Conclusion Acoustic analysis is the measure used in most studies to verify the effect of the intervention in vocally healthy individuals.

RESUMO Objetivo mapear as medidas de avaliação vocal utilizadas para verificar o efeito da intervenção em indivíduos vocalmente saudáveis. Estratégia de pesquisa trata-se de uma revisão de escopo baseada na questão de pesquisa: “Quais as medidas de avaliação vocal utilizadas para verificar o efeito da intervenção em indivíduos vocalmente saudáveis?” A busca foi realizada de forma eletrônica nas bases de dados MEDLINE (PubMed), LILACS (BVS), Scopus (Elsevier), Web of Science (Clarivate), Embase e Cochrane. Critérios de seleção a seleção dos estudos foi baseada na leitura dos títulos, resumos, palavras-chave e textos completos, aplicando-se os critérios de elegibilidade. Foram extraídos os dados relacionados às informações bibliográficas da publicação, características da amostra e da intervenção, os efeitos da intervenção nas medidas de autoavaliação, perceptivo-auditivas, acústicas, aerodinâmicas, eletroglotográficas, resultado do exame laríngeo, entre outros. Os dados foram resumidos e apresentados de forma quantitativa e descritiva. Resultados foram selecionados 97 artigos, entre os 979 estudos mapeados nesta revisão. A análise acústica foi a medida mais utilizada (n=70, 72,3%) para verificar os efeitos do treinamento vocal nos estudos selecionados, seguida pela eletroglotografia (n= 55, 56,7%), autoavaliação (n= 38, 39,2%), aerodinâmica (n= 33, 34,0%), julgamento perceptivo-auditivo (n= 22, 22,7%) e exame laríngeo (n= 16, 16,5%). Conclusão a análise acústica é a medida utilizada na maioria dos estudos para verificar o efeito da intervenção em indivíduos vocalmente saudáveis.

RESUMO No XXVIII Congresso Brasileiro da SBFa, 24 especialistas reuniram-se e, a partir de um posicionamento condutor sobre pesquisa científica como ferramenta de conexão entre laboratório e clínica, cinco frentes de conhecimento da especialidade de voz foram discutidas: 1. Julgamento perceptivo-auditivo da qualidade vocal; 2. Análise acústica do sinal vocal; 3. Autoavaliação em voz; 4. Técnicas tradicionais de terapia; 5. Técnicas modernas de eletroestimulação e fotobiomodulação em voz. A parte “a” desta publicação é a consolidação das análises dos três primeiros aspectos. A tendência no julgamento perceptivo-auditivo da qualidade vocal é o uso de protocolos padrão. A avaliação acústica do sinal vocal é acessível e pode ser feita de modo descritivo ou por extração de parâmetros, preferindo-se medidas multiparamétricas. Finalmente, a análise do próprio indivíduo fecha essa tríade de documentação fonoaudiológica, que será base para a conclusão da avaliação, referência para monitoramento do progresso e avaliação de resultado de tratamento.

ABSTRACT During the XXVIII Brazilian Congress of SBFa, 24 specialists met and, from a leading position on scientific research as a tool for connecting laboratory and clinic, five fronts of knowledge of the voice specialty were discussed as following: Perceptual-auditory judgment of vocal quality; 2. Acoustic analysis of the vocal signal; 3. Voice self-assessment; 4. Traditional techniques of therapy; 5. Modern techniques of electrostimulation and photobiomodulation (PBMT) in voice. Part “a” of this publication was associated with the consolidation of the analyses of the first three aspects. The trend in the perceptual-auditory judgment of vocal quality was related to the use of standard protocols. The acoustic evaluation of the vocal signal is accessible and can be done descriptively or by extraction of parameters, thus preferring multiparametric measures. Finally, the analysis of the individual himself closes this triad of voice documentation, which will be the basis for the conclusion of the evaluation, reference for monitoring progress, and evaluation of treatment results.

RESUMO Objetivo Identificar os termos referidos pela população em geral para a qualidade vocal saudável, rugosa e soprosa. Métodos foi realizado um teste, de modo presencial, com 50 participantes sem vínculos acadêmicos ou profissionais com a Fonoaudiologia. A tarefa consistia em ouvir três vozes e defini-las livremente. A primeira voz apresentada era predominantemente soprosa; a segunda, predominantemente rugosa e a terceira, vocalmente saudável. Apresentou-se a emissão sustentada da vogal /Ɛ/ e a contagem de 1 a 10. Cada participante deveria responder ao comando: “Ouça essa voz. Com qual termo você a nomearia?”, digitando a resposta em uma linha disposta na tela do PowerPoint. Resultados para a voz saudável, o termo que mais se repetiu foi “normal” (36%); outros termos foram: “limpa”, “comum”, “padrão”, “clara”, “límpida”, “firme”, “boa”, “som aberto”, “definida”. Para a voz rugosa, 25 participantes (50%) responderam com o termo “rouca” e os demais se dividiram em termos como “ruidosa”, “chiada”, “voz de fumante”, “grave”, “idosa”, “cavernosa”, “anormal”, entre outros termos similares. Para a voz soprosa, 24 participantes (48%) usaram o termo “cansada”; cinco atribuíram o adjetivo “fraca”; três responderam com o termo “sem fôlego”; houve duas correspondências aos termos “arrastada” e “doente” e os demais participantes responderam com termos semelhantes: “exausta”, “preguiçosa”, “sonolenta”, “fatigada” e afins. Conclusão os termos “normal” para voz saudável, “rouca” para voz rugosa e “cansada” para voz soprosa possibilitam a percepção mais usual desses parâmetros clínicos de qualidade vocal, para indivíduos alheios à linguagem técnico-científica da Fonoaudiologia

ABSTRACT Purpose Identify the terms mentioned by the general population for healthy, rough and breathy vocal quality. Methods A test was carried out with 50 participants, in person, without academic or professional ties with Speech Therapy. The task was to hear three voices and define them freely. The first voice presented was predominantly breathy; the second, predominantly rough and the third, vocally healthy. The sustained emission of the vowel / Ɛ / and the count from one to ten were presented. Each participant should respond to the command: “Listen to that voice. Which term would you name it?”, Typing the answer on a line displayed on the PowerPoint screen. Results For the healthy voice, the term that was repeated the most was “normal” (36%), other terms were: “clean”, “common”, “standard”, “clear”, “clear”, “firm”, “good”, “open sound”, “defined”. For the rough voice, twenty-five participants (50%) responded with the term “hoarse” and the others were divided into terms such as “noisy”, “smoker's voice”, “deep”, “elderly”, “cavernous”, “abnormal”, among other similar terms. For the breathy voice, twenty-four participants (48%) used the term “tired”; five participants assigned the adjective “weak”; three responded with the term “out of breath”; there were two correspondences to the terms “dragged” and “sick”; and the other participants responded with terms similar: “exhausted”, “lazy”, “sleepy”, “fatigued” and the like. Conclusion The terms “normal” for a healthy voice, “hoarse” for a rough voice and “tired” for a breathy voice, allow a more usual perception of these clinical parameters of vocal quality, for individuals outside the technical-scientific language of Speech Therapy.

RESUMO Objetivo Investigar o efeito imediato da vibração sonorizada de língua (VSL), do canudo de alta resistência no ar (CAR) e da sobrearticulação (SA) sobre o espaço vocálico de mulheres vocalmente saudáveis (MVS) e com nódulos vocais (MNV). Método Participaram 12 mulheres no MNV e 12 mulheres no MVS, alocadas para execução dos exercícios vocais de VSL, CAR e AS. Cada participante realizou apenas um dos três exercícios propostos, durante 5 minutos, antecedidos e sucedidos pela gravação de uma sequência de frases-veículo para extração dos formantes (F1 e F2) dos segmentos vocálicos [a, i, u]. O espaço vocálico foi analisado por meio das diferenças entre as medidas dos formantes das vogais. Resultados houve redução de F1 no intervalo [a]-[i] e [i]-[u] e de F2 entre as vogais [a]-[u] e [i]-[u] no MVS, após realização do CAR. No MNV, observou-se redução de F2 no intervalo [a]-[i] após VSL. Na análise intergrupo, houve maiores valores de F1 entre os intervalos das vogais [a]-[i] e [i]-[u] no MVS, antes da realização do CAR, e após exercício apenas no intervalo [a]-[i]. Observou-se maior valor de F1 e F2 no intervalo entre as vogais [i]-[u] no MNV após VSL. Conclusão O exercício de VSL diminuiu o espaço vocálico em mulheres do MNV. O CAR reduziu o espaço vocálico de mulheres do MVS. O MNV apresentou menor espaço vocálico em relação ao MVS, antes e após a realização do CAR. Houve redução do espaço vocálico no MNV em relação ao MNV após o exercício de VSL.

ABSTRACT Purpose To investigate the immediate effect of voiced tongue vibration (VSL), high-resistance straw in the air (CAR), and overarticulation (OA) on the vocal space of vocally healthy women (MVS) and with vocal nodules (MNV). Methods 12 women participated in the MNV and 12 women in the MVS, allocated to perform the vocal exercises of VSL, CAR, and OA. Each participant performed only one of the three proposed exercises, for 5 minutes, preceded and followed by recording a sequence of vehicle sentences for extracting formants (F1 and F2) from the vowel segments [a, i, u]. The vowel space was analyzed through the differences between the measures of the formants of the vowels. Results we observed a reduction of F1 in the interval [a]-[i] and [i]-[u] and of F2 between the vowels [a]-[u] and [i]-[u] in the MVS, after performing the CAR. In MNV, we observed a reduction of F2 in the interval [a]-[i] after VSL. In the intergroup analysis, there were higher F1 values between the intervals of the vowels [a]-[i] and [i]-[u] in the MVS, before performing the CAR, and after exercise only in the interval [a]-[i]. A higher value of F1 and F2 was observed in the interval between the vowels [i]-[u] in the MNV after VSL. Conclusion The VSL exercise reduced the vowel space in MNV women. CAR reduced the vocal space of women in the MVS. The MNV had a smaller vowel space compared to the MVS before and after the CAR. We observed a reduction in the vowel space in the MNV compared to the MNV after the VSL exercise.

RESUMO O presente texto é a continuação da publicação referente ao XVIII Congresso da SBFa. Na parte “A” apresentamos análises sobre avaliação clínica vocal. O foco da parte “B” são aspectos de reabilitação vocal: 4. Técnicas tradicionais de terapia vocal; 5. Técnicas modernas de eletroestimulação e fotobiomodulação aplicadas à reabilitação. Os inúmeros estudos sobre os diversos programas, métodos ou técnicas tradicionais de reabilitação, muitos de elevada qualidade de evidência, permitem considerar tais procedimentos relativamente bem descritos, seguros e com efeitos conhecidos, dando conta do tratamento de diversos distúrbios vocais. As evidências científicas com as técnicas tradicionais são reconhecidas mundialmente. Novas frentes de evolução, como o uso da eletroestimulação ou fotobiomodulação em voz parecem ser promissoras como abordagens coadjuvantes. Há mais estudos sobre eletroestimulação em voz do que com fotobiomodulação, contudo, evidências científicas para essas duas técnicas modernas são ainda limitadas. Conhecimento e cautela são necessários para a aplicação de quaisquer técnicas.

ABSTRACT This text is the continuation of the XVIII SBFa Congress publication. In part “A” we presented the analyses on clinical vocal evaluation. Part “B” focuses on vocal rehabilitation: 4. Traditional techniques of vocal therapy; 5. Modern techniques of electrostimulation and photobiomodulation applied to vocal rehabilitation. The numerous studies on the various programs, methods, and techniques of traditional rehabilitation techniques, and many with high quality of evidence, allow us to consider such procedures relatively well described, safe, and with known effects, accounting for the treatment of various vocal disorders. The scientific evidence with traditional techniques is recognized worldwide. New fronts of evolution, with electrostimulation or photobiomodulation used to handle voice problems, seem to be promising as coadjutant approaches. There are more studies on electrostimulation in vocal rehabilitation than with photobiomodulation; however, scientific evidence for these two modern techniques is still limited. Knowledge and caution are required for the application of either technique.

RESUMO Objetivo caracterizar e refletir sobre o processo de equivalência cultural e/ou validação de instrumentos de autoavaliação vocal traduzidos e adaptados para o português brasileiro, utilizados na prática clínica e em pesquisas científicas. Estratégia de pesquisa Buscas realizadas nas bases de dados: SciELO, LILACS, PubMed e The Cochrane Library. Critérios de seleção estudos selecionados quanto à presença dos descritores citados em seu título, resumo ou lista de descritores; artigos de validação de instrumento de autoavaliação em voz, ou com o objetivo de realizar equivalência cultural; direcionados à população brasileira; estudos originais com amostra de seres humanos, independentemente da idade do ciclo vital, tipo de disfonia ou sintomas vocais. Resultados nove artigos selecionados. Qualidade de vida em voz e desvantagem vocal foram os construtos mais abordados. A maioria das validações ocorreu na Região Sudeste do Brasil. A maior parte dos escores dos instrumentos foi calculada por somatório simples das respostas dos participantes nos itens; pontos de corte nem sempre foram apresentados. Os domínios dos instrumentos, bem como os itens que os compunham, foram mantidos conforme apresentado no instrumento em sua língua original. A estatística mais comum para análise dos instrumentos foi o coeficiente Alfa de Crombach. Conclusão os construtos mais abordados nos artigos selecionados foram qualidade de vida em voz e índice de desvantagem vocal. Os instrumentos foram considerados válidos e sensíveis para autoavaliação vocal, mesmo quando não consideradas as etapas propostas internacionalmente para validação de instrumentos em saúde.

ABSTRACT Objective to characterize and reflect on the process of cultural equivalence and/or validation of vocal self-assessment instruments translated and adapted to the Brazilian Portuguese, used in clinical practice and scientific research. Research strategy The search of articles was carried out in the following databases: SciELO, LILACS, PubMed and “The Cochrane Library”. Selection criteria The studies were selected regarding the presence of the descriptors cited in their title, abstract or in the list of descriptors; validation articles of a self-assessment instrument in voice or with the objective of performing cultural equivalence; to be directed to the Brazilian population; original studies with a sample of human beings; regardless of the age of the life cycle, type of dysphonia or vocal symptoms. Results Nine articles were selected. Quality of life in voice and vocal handicap were the most addressed constructs. Most validations occurred in the Southeast region of Brazil. The scores of the instruments is calculated by a simple sum of the answers given by the participants in the items; cutoff points are not always presented. The domains of the instruments, as well as the items that compose them, were maintained as presented in the instrument in its original. The most common statistic for instrument analysis was Cronbach's Alpha coefficient. Conclusion The constructs most addressed in the selected articles were quality of life in voice and vocal handicap index. The instruments were considered valid and sensitive for vocal self-assessment, even not considering the international proposals for validation of health instruments.

RESUMO Objetivo Avaliar a efetividade da terapia de grupo na redução dos sintomas vocais em pacientes com disfonia comportamental e compará-la a uma modalidade de terapia tradicional/individual. Métodos Trata-se de uma pesquisa explicativa, quantitativa e de intervenção. Participaram 99 pacientes com disfonia comportamental, de ambos os sexos, alocados em dois grupos: Terapia Individual (TI) e Terapia de Grupo (TG). Todos foram submetidos à terapia com abordagem eclética. Foram realizadas oito sessões, sendo a primeira e a última destinadas à avaliação e as outras seis de intervenção. Realizou-se análise estatística descritiva e inferencial para comparar os grupos e os momentos pré e pós-intervenção. Resultados A maioria dos participantes era do sexo feminino, não usava a voz profissionalmente e com lesão na porção membranosa da prega vocal. Ambos os grupos apresentavam escores médios semelhantes, no início da terapia, fato que mostra a homogeneidade dos grupos. Houve redução de todos os escores da Escala de Sintomas Vocais (ESV) no momento pós-intervenção individual e em grupo. Não foi observada diferença significativa, ao comparar as médias dos escores da ESV pós-terapia entre os grupos. Em relação aos itens da ESV, foi possível observar que alguns podem não detectar as diferenças entre os momentos pré e pós-intervenção. Conclusão Tanto a TI, quanto a TG foram efetivas na redução significativa dos sintomas vocais. Não houve diferença ao comparar os grupos. Alguns itens da ESV, principalmente no domínio limitação, foram mais sensíveis nos momentos pós-intervenção, em ambas modalidades. O tipo de intervenção influencia a redução dos sintomas vocais.

ABSTRACT Purpose To evaluate the effectiveness of two therapeutic approaches in reducing vocal symptoms in patients with behavioral dysphonia. Methods This was an explanatory, quantitative and interventional study. A total of 99 patients of both sexes with behavioral dysphonia who sought speech therapy participated in this study. These patients were allocated into two groups: individual therapy (IT) and group therapy (GT). All participants were subjected to therapy with an eclectic approach. Eight sessions were conducted, comprising a first and a last session for evaluation, with six intervention sessions in between them. A descriptive and inferential statistical analysis was performed to compare the groups and the moments before and after the intervention. Results The majority of participants were female, and there was a predominance of patients who did not use their voice professionally and who had a diagnosis of a laryngeal lesion in the membranous portion of the vocal fold. It should be noted that patients presented similar mean scores at the beginning of therapy regardless of the group to which they were allocated, which indicated the homogeneity of the groups. There were reductions in all Vocal Symptoms Scale (VoiSS) scores after individual and group therapy. No significant differences were observed when comparing the mean posttherapy VoiSS scores between the groups in either domain. Some VoiSS items were unable to detect differences between the pre- and posttherapy timepoints. Conclusion Individual and group therapeutic modalities are effective in significantly reducing self-reported vocal symptoms. The type of intervention influences the reduction in vocal symptoms. Some items of the VoiSS, mainly in the area of limitations, were more sensitive at the posttherapy timepoint in both modalities.

Resumo Introdução: O zumbido é um sintoma auditivo de natureza subjetivo, normalmente associado a um som, mesmo na ausência de fontes sonoras externas. Apresenta diagnóstico complexo. Uma das formas de mensuração é o uso, isolado ou em conjunto, de questionários de autoavaliação, como o Tinnitus Handicap Inventory, a escala visual analógica e o exame de acufenometria. Objetivo: Analisar a correlação entre os métodos de mensuração do zumbido: Tinnitus Handicap Inventory, escala visual analógica e acufenometria. Método: Participaram do estudo 148 pacientes com queixa de zumbido crônico. Realizou-se avaliação otorrinolaringológica, anamnese direcionada ao zumbido, audiometria (tonal e vocal), imitanciometria, Tinnitus Handicap Inventory, escala visual analógica e acufenometria. O estudo foi registrado no comitê de ética da instituição, com nº 0129/12. Resultados: Em relação à frequência das respostas do Tinnitus Handicap Inventory, foi observada maior ocorrência do grau leve. Observou-se uma média de 6 pontos na escala visual analógica. A média da sensação de intensidade medida na acufenometria na orelha direita foi de 20 dBNS e na orelha esquerda foi de 17 dBNS. Quanto ao tipo de estímulo, o mais encontrado foi o tom puro contínuo. A sensação de frequência, no maior número de casos, foi de 6.000 Hz Entre as medidas do Tinnitus Handicap Inventory e da escala visual analógica, observou-se correlação significante. À medida que um valor aumenta o outro também aumenta. Os resultados da acufenometria e da escala visual analógica também apresentam significância. Conclusão: Houve correlação significante entre os valores medidos pelo Tinnitus Handicap Inventory, escala visual analógica (incômodo) e acufenometria (loudness) na avaliação do zumbido. A escolha de um dos três métodos avaliativos para pesquisa do zumbido fornece dimensões diferentes do zumbido e se complementam.

Abstract Introduction: Tinnitus is a subjective auditory symptom usually associated with a sound, even in the absence of external sound sources. Its diagnosis is complex, and some of the forms of measurement alone or in combination, include self-assessment questionnaires, such as the tinnitus handicap inventory, the visual analog scale and/or pitch and loudness matching. Objective: To analyze the correlation among three tinnitus measurement methods: tinnitus handicap inventory, visual analog scale and pitch and loudness matching. Methods: The study consisted of 148 patients complaining of chronic tinnitus. An otorhinolaryngological evaluation, anamnesis directed to tinnitus, audiometry (pure tone and speech), imitanciometry, tinnitus handicap inventory, visual analog scale, and pitch and loudness matching were performed. The study was registered in the Ethics Committee of the Institution with no. 0129/12. Results: Regarding the frequency of tinnitus handicap inventory responses, a higher occurrence of the mild degree was observed. An average of 6 points was observed on the visual analog scale. The mean loudness matching in the right ear was 20 dBNS, and in the left ear was 17 dBNS. As for the type of stimulus, the most found was continuous pure tone. The frequency of the pitch sensation was 6000 Hz in the largest number of cases. Regarding the measures of tinnitus handicap inventory and the visual analogical scale, a significant correlation was observed, and as one value increases the other also increases. Pitch and loudness matching and the visual analogical scale results are also significant. Conclusion: There was a significant correlation between the values measured by the tinnitus handicap inventory, visual analogical scale (annoyance) and loudness matching in the evaluation of tinnitus. The selection of any one of the three evaluative methods for tinnitus investigation provides different dimensions of the tinnitus and complements the others.

Abstract Introduction Dichotic listening refers to the ability to hear different sounds presented to each ear simultaneously. Objective The aim of the present study was to assess dichotic listening in women throughout the menstrual cycle. Methods The volunteers who met the eligibility criteria participated in a dichotic listening assessment composed of three tests: 1) staggered spondaic word test; 2) dichotic digits test; and 3) consonant-vowel test. The female participants were tested during two different phases of the menstrual cycle: the follicular (days 11 to 13) and luteal (days 23 to 26) phases. The phases were confirmed by measuring serum levels of the hormone estradiol. Results A total of 20 volunteers aged 18 to 49 years participated in the study (9 females and 11 males). In test 1, only the right ear of females showed better performance during the follicular phase (high estrogen levels), compared with the luteal phase (low estrogen levels); in test 2, there were no significant differences for any of the groups; and in test 3, both males and females showed significantly better performance in their right ear compared with their left ear. Conclusion The better performance of females during the follicular phase of the cycle may indicate that estrogen levels might have an influence on dichotic listening in women.

RESUMO Objetivo Analisar a configuração acústico-articulatória das vogais em mulheres com nódulos vocais e vocalmente saudáveis. Método Participaram do estudo 12 mulheres com nódulos vocais (GE) e 12 vocalmente saudáveis (GC). Todas as mulheres gravaram frases-veículo com as vogais /a/, /i/ e /u/ em posição tônica, sucedidas e precedidas da oclusiva /p/: “Digo papa baixinho”, “Digo pipa baixinho” e “Digo pupa baixinho”. Posteriormente, foram extraídos os três primeiros formantes (F1, F2 e F3) dessas vogais. Resultados Observou-se diferença nas medidas de F1 para as vogais /a/ e /u/ e F2 para a vogal /a/ entre os dois grupos estudados. Mulheres com nódulos vocais apresentam menor valor dessas medidas em relação às mulheres vocalmente saudáveis. Pacientes com nódulos vocais apresentaram menor intervalo nos valores de F1 e F2 entre as vogais /a/, /i/ e /u/ em relação às mulheres vocalmente saudáveis. Conclusão Mulheres com nódulos vocais apresentam menores valores de F1 e F2, e menor amplitude de movimentação dos articuladores na produção vocálica em relação às mulheres vocalmente saudáveis.

ABSTRACT Purpose To analyze the acoustic-articulatory configuration of vowels in women with vocal nodules and with healthy voice. Methods Twelve women with vocal nodules (EG) and twelve vocally health women (CG) participated of this study. All women recorded vehicle phrases with the vowels /a/, /i/, and /u/ in stress position, preceded and followed by the occlusive consonant /p/: “Digo papa baixinho”, “Digo pipa baixinho”, and “Digo pupa baixinho”. Subsequently, the first three formants (F1, F2, and F3) were extracted from these vowel targets. Results Between the two groups studied, F1 measures differed for vowels /a/ and /u/, and F2 measures differed for the vowel /a/. Women with vocal nodules showed lower values for these measures compared to vocally healthy women. Patients with vocal nodules showed a smaller interval in F1 and F2 values between vowels /a/, /i/, and /u/ compared to vocally healthy women. Conclusion Women with vocal nodules show lower F1 and F2 values and lower range of motion of the articulators during vowel production compared to vocally healthy women.

RESUMO Objetivo Analisar se existe associação entre a presença, a intensidade e o tipo de desvio vocal e as medidas cepstrais em amostras de indivíduos com queixa vocal. Método Foram utilizadas 376 amostras da vogal /ε/ de indivíduos de ambos os gêneros, com queixa vocal. Utilizou-se uma escala analógico-visual para análise perceptivo-auditiva das vozes quanto à intensidade do desvio vocal (GG), graus de rugosidade (GR), soprosidade (GS) e tensão (GT), incluindo-se a determinação da qualidade vocal predominante (rugosa, soprosa ou tensa). Foram extraídas as medidas relacionadas ao Cepstral Peak Prominence-Smoothed (CPPS) e o declínio espectral das amostras vocais. Resultados Houve diferença dos valores do CPPS entre os grupos com e sem desvio vocal, assim como entre as diferentes intensidades e tipos de desvio vocal. Os valores do CPPS foram mais reduzidos em função da presença e intensidade do desvio vocal. Os valores do CPPS diferenciaram vozes rugosas x soprosas, rugosas x tensas e soprosas x tensas. O declínio espectral apenas diferenciou vozes soprosas x tensas. O CPPS se correlacionou de modo positivo e forte com os GG e GS, de modo negativo moderado com o GR, e de forma negativa fraca com o GT. O declínio espectral apresentou correlação positiva moderada com o GT e correlação negativa fraca com o GS. Conclusão Existe associação entre a presença de desvio vocal, o GG, a qualidade vocal predominante e o CPPS. De modo especial, o GG é fortemente correlacionado ao CPPS. O declínio espectral está associado apenas aos parâmetros de soprosidade e tensão.

ABSTRACT Purpose To analyze whether there is an association between the presence, intensity and type of voice disorder and the cepstral measures in samples of individuals with voice complaints. Methods We used 376 vowel /Ɛ/ samples from individuals of both genders that had voice complaints. An analogue-visual scale was used for the auditory-perceptual analysis of voices regarding the overall grade of dysphonia (G) and the grades of roughness (R), breathiness (B), and strain (S), including a determination of voice quality (rough, breathy or strained). Measures related to cepstral peak prominence smoothed (CPPS) and spectral decline of vocal samples were extracted. Results There were differences in the CPPS values between the groups with or without voice disorders as well as between the different intensities and types of voice disorder. CPPS values were lower because of the presence and intensity of voice disorders. The CPPS values differentiated the following voices: rough x breathy, rough x strained, and breathy x strained. The spectral decline only differentiated breathy x strained voices. CPPS correlated positively and strongly with G and B; moderately and negatively with R, and negatively and weakly with S. The spectral decline had a moderate positive correlation with S and a weak negative correlation with B. Conclusion There is association between voice disorder, G, predominant voice quality, and CPPS. In particular, G is strongly correlated with CPPS. Spectral decline is associated only with the parameters B and S.

RESUMO Objetivo Identificar se existe correlação entre o Índice de Triagem para Distúrbio de Voz e as condições de trabalho de professores das escolas públicas e privadas e compará-las entre os professores das duas redes de ensino. Métodos Participaram 183 professores, sendo 122 de escolas públicas e 61 de uma escola privada, que responderam ao questionário Condição de Produção Vocal do Professor, composto por 81 questões. Destas, entretanto, foram analisadas as questões referentes à identificação pessoal, funcional, condições vocais e de trabalho. Foi realizada análise estatística descritiva e inferencial. Resultados Quando analisados em relação às condições vocais, a maioria dos professores das escolas públicas relatou queixa vocal. O Índice de Triagem para Distúrbio de Voz dos professores das escolas públicas correspondeu a mais sintomas vocais do que os da escola privada. Além disso, os docentes das escolas públicas relataram piores condições de trabalho, em relação aos docentes da escola privada. Conclusão Existe uma correlação negativa entre o Índice de Triagem para Distúrbio de Voz e as condições de trabalho dos professores de escola pública e particular. Os professores das escolas públicas relataram piores condições de trabalho do que os da escola privada, bem como referiram maior ocorrência de distúrbio da voz e apresentaram maior pontuação no Índice de Triagem para Distúrbio de Voz.

ABSTRACT Purpose To identify whether there is a correlation between the Screening Index for Voice Disorder and working conditions of public and private school teachers, and compare them with teachers from both groups. Methods 183 teachers participated in the study, 122 from public schools and 61 from a private school. They responded to the questionnaire Teacher’s Vocal Production Condition, which consists of 81 questions. However, only the questions related to personal information, functional situation, and vocal and working conditions were analyzed. Descriptive and inferential statistical analyses were performed. Results In terms of voice conditions, most public school teachers reported voice complaints. The Screening Index for Voice Disorder showed more voice symptoms of public school teachers than of private school teachers. Furthermore, the public school teachers reported worse working conditions than the private school teachers. Conclusion There is a negative correlation between the Screening Index for Voice Disorder and the working conditions of the public and private school teachers. Comparing with the private school teachers, the public school teachers reported worse working conditions, more prevalence of voice disorders and higher scores in the Screening Index for Voice Disorder.

Resumo Introdução Os distúrbios de voz alteram o sinal sonoro de diversas formas, combinam variados tipos de perturbações e ruídos na emissão vocal. O diagrama de desvio fonatório é um gráfico bidimensional que possibilita a avaliação do sinal vocal a partir da combinação de medidas de periodicidade (jitter, shimmer e coeficiente de correlação) e de ruído (Glottal to Noise Excitation). O uso de sinais sintetizados, no qual se tem um maior controle e conhecimento das condições de produção, pode possibilitar uma maior compreensão dos mecanismos fisiológicos e acústicos subjacentes à emissão vocal e seus principais correlatos perceptivo-auditivos quanto à intensidade do desvio e aos tipos de qualidade vocal. Objetivo Analisar o desempenho do diagrama de desvio fonatório na discriminação da presença e do grau de rugosidade e soprosidade em vozes sintetizadas. Método Foram utilizados 871 sinais vocais sintetizados correspondentes à vogal/ɛ/. Realizou-se a análise perceptivo-auditiva do grau de rugosidade e soprosidade dos sinais sintetizados, com uma escala visual analógica. Posteriormente, os sinais foram categorizados quanto à presença/ausência desses parâmetros a partir dos valores de corte da escala visual analógica. A análise acústica foi realizada por meio da avaliação da distribuição dos sinais vocais de acordo com a área, quadrante, forma e densidade do diagrama de desvio fonatório. Executou-se o teste de igualdade de proporções e o teste qui-quadrado para comparar as variáveis. Resultados Sinais vocais rugosos e soprosos localizaram-se predominantemente fora da área de normalidade e no quadrante inferior direito do diagrama de desvio fonatório. Vozes com maiores graus de rugosidade e soprosidade localizaram-se fora da área de normalidade, no quadrante inferior direito e apresentaram densidade concentrada. Conclusão A área de normalidade e o quadrante do diagrama de desvio fonatório são capazes de discriminar vozes saudáveis de rugosas e soprosas. Vozes com maior grau de rugosidade e soprosidade localizam-se proporcionalmente fora da área de normalidade, no quadrante inferior-direito e com densidade concentrada.

Abstract Introduction Voice disorders alter the sound signal in several ways, combining several types of vocal emission disturbances and noise. The phonatory deviation diagram is a two-dimensional chart that allows the evaluation of the vocal signal based on the combination of periodicity (jitter, shimmer, and correlation coefficient) and noise (Glottal to Noise Excitation) measurements. The use of synthesized signals, where one has a greater control and knowledge of the production conditions, may allow a better understanding of the physiological and acoustic mechanisms underlying the vocal emission and its main perceptual-auditory correlates regarding the intensity of the deviation and types of vocal quality. Objective To analyze the performance of the phonatory deviation diagram in the discrimination of the presence and degree of roughness and breathiness in synthesized voices. Methods 871 synthesized vocal signals were used corresponding to the vowel /ɛ/. The perceptual-auditory analysis of the degree of roughness and breathiness of the synthesized signals was performed using visual analogue scale. Subsequently, the signals were categorized regarding the presence/absence of these parameters based on the visual analogue scale cutoff values. Acoustic analysis was performed by assessing the distribution of vocal signals according to the phonatory deviation diagram area, quadrant, shape, and density. The equality of proportions and the chi-square tests were performed to compare the variables. Results Rough and breathy vocal signals were located predominantly outside the normal range and in the lower right quadrant of the phonatory deviation diagram. Voices with higher degrees of roughness and breathiness were located outside the area of normality in the lower right quadrant and had concentrated density. Conclusion The normality area and the phonatory deviation diagram quadrant can discriminate healthy voices from rough and breathy ones. Voices with higher degrees of roughness and breathiness are proportionally located outside the area of normality, in the lower right quadrant and with concentrated density.
